Criminal Offender Record Information forms, known as CORI, are gathered from paid staff and volunteers each year. There are a few ways in which volunteers can help with the process. You can receive a form from a Coordinator in your ministry, pick one up at the Parish Center, or download a FY2016 CORI Parish Form. (Print single-sided, please)

Please fill out this important document and return it to the Parish Center. The CORI coordinator is available Monday-Friday, 8:30 a.m. to 4:30 p.m. Please note that a photo ID and the last six digits of your Social Security Number are required for the form. (A Social Security Card or Birth Certificate is accepted alternatives if no ID is available.)
The Archdiocese of Boston requires annual CORI renewals for every volunteer, priest, religious, and employee in area parishes. The screening document is a requirement to secure the safety of our children, youth, elderly, and the disabled. More than 55,000 CORIs are processed by the RCAB every year.
